Registered Nurse (RN) Position
Job Details
Responsibilities:

Wanting to join a vibrant and dynamic team and to work with a well-respected company in the MD/DC Metro area.

Meets new and prospective clients ( inside and outside the community) and performs an initial assessment of service needs - This could be done virtual and/or in-person, depending on the client's request.

Ability to perform Wellness checks.
-
Ability to participate in wellness and educational sessions for CWC clients, caregivers and other interested members (if this is something you are interested in).
-
Develops a Plan of Care for all new clients and updates as needed.
-
Communicates with care managers, caregivers, office staff, clients, family members and other medical professionals about client's needs and concerns.
-
Consults with the Director of Nursing regarding the supervision of services.
-
Provides training to the clients regarding their care, when requested.
-
Assures proper maintenance of clinical records and documentation in compliance with State and Federal laws.
-
Performs client & caregiver evaluations every 30, 90 or 120 days as needed.
-
Trains new staff and existing staff.
-
Enjoys working with a warm and supportive team.
Qualifications:
-
RN License – Compact license (MD or VA) OR DC License
-
Current and valid CPR / BLS Certification
-
TB vaccination, Full COVID vaccination and MUST BE BOOSTED and proof of Hep B vaccination.
-
Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
-
Clear criminal background and driving record
-
Self-starter with the ability to work independently and manage time effectively
-
Strong interpersonal skills, including a willingness to teach and mentor caregivers
-
Comfortable with electronic records and MS Office Suite
Licensed Practice Nurse (LPN) Position

Caregiver (PCA) Position
Job Details
Responsibilities:
-
Assist with daily activities to help clients stay independent and in their own homes.
-
Provide assistance with personal care.
-
Assist with mobility, transfers and range of motion exercises.
-
Handle meal planning, preparation and feeding.
-
Properly manage household needs.
-
Provide companionship and respite services for the family.
